The purpose of this form is to provide parents, coaches, and players within North Toronto’s recreational program an opportunity to anonymously give feedback to the Club about the conduct and standard of match officials at games.
This form is not intended to be a sounding board for opinions on the calls an official has made. We will not consider these opinions and comments.
On the contrary, we encourage the use of this form to highlight excellent match officials as well as to make us aware of any problematic behaviour you observe on and off the field.
