Players interested in trying out for our Ontario Premier League teams need to complete the Register Your Interest Form and submit a Trial Permit Application, Short Term Registration Permit, or International Trialist Permit Form for approval. Please see the forms below:
Permission Forms:
- Trial Permit Application – For players already registered to another organization within Ontario.
- Short Term Registration Permit – For players not currently registered with any organization
- International Trialist Permit Form (ITPF) – For players last registered outside of province or outside of country.
Permission forms must be uploaded when completing your “Register Your Interest Form”. Once selected for a tryout, your forms will be sent for approval.
